Permission to Skate Forms have been released by OMHA and are available at the links below.
Please select the appropriate form for the age group of your child.
Please fill in and return the PDF file to Adam Armaly - VP of Travel at [email protected].
A signed copy will be returned to you as soon as possible.
NOTE: These forms are NOT required to be eligible for the AAA 'development skates' being offered, but only required for the formal tryouts.
