    AMHA FUND RAISING REQUEST FORM
    This form is to be submitted to the V.P of Travel 7 days before the scheduled monthly board meeting and 21 days before an event takes place. A full accounting of each fundraising event is required to be submitted to the Vice President (Division) on the prescribed form within 14 days of holding or substantially completing the event. Only one fundraising event per form.
