Birth Certificates and Medical Forms
If your child is a first year Registrant with AMHA we require a copy of their Birth Certificate.
Additionally, if you have not yet provided a copy of the completed Medical Form, your Team's Trainer will require this. The deadline for submission of both these documents is Sunday, November 3, 2013.
Either document can be submitted to myself or your Child's Coach.
